הצורך בהגדרה אחידה של דיסקלקוליה


מי שקרא את המצגת "לקות למידה – סיפורה של הגדרה" עמד על הקשיים הנגרמים לילדים עצמם ולהתפתחות המחקר, כתוצאה מחוסר הסכמה בין מומחים על כל אחד (כמעט) מהמאפיינים הבסיסיים של לקות למידה.

מאמר זה מנסה לחתור להגדרה אחידה של דיסקלקוליה התפתחותית.

המאמר מציג שלוש גישות לדיסקלקוליה, אותן אביא מתוך נקודת המבט שלי ועם 
הפרשנות שלי:

1.  דיסקלקוליה התפתחותית קשורה לחסכים בסיסיים ב – NUMBER SENSE.  כאן מדובר בקבוצת ילדים שיש להם קושי בהבנת המשמעות הכמותית של המספר.  ילדים כאלה הם איטיים, למשל, במשימה הדורשת לקבוע אם כמות נקודות במקבץ זהה לערך ספרה, כאשר הספרות והכמויות הן עד 9.   החוקר BUTTERWORTH טוען שקושי זה, שהוא כל כך ספציפי, מצביע על קיומה של מודולה מספרית, שהיא פגועה אצל ילדים עם דיסקלקוליה.

2.  קיימים תת סוגים של דיסקלקוליה התפתחותית הנגרמים מפגיעה בתהליכים קוגניטיבים שונים.  פגיעה בזיכרון עבודה מילולי, בזיכרון סמנטי, בעיבוד חזותי מרחבי וביכולת הפלואידית משפיעים על התפקוד במתמטיקה.  אני מעדיפה לא לכנות קבוצה זו בשם "דיסקלקולים", אלא  - פשוט - "לקויי למידה".  זאת מכיוון שהלקויות שיש להם ביכולות הקוגניטיביות השונות (עיבוד חזותי, זיכרון לטווח קצר, ידע מגובש וכו' ) פוגעות מן הסתם לא רק במתמטיקה אלא גם בקריאה, כתיבה והבנת הנקרא.  כל אחד מהילדים הללו הוא לקוי למידה באופן  שונה (בהתאם ליכולת הקוגניטיבית הספציפית הפגועה אצלו) וכך התפקוד שלו בחשבון (וגם בקריאה, כתיבה והבנת הנקרא) יהיה שונה, לדעתי.

3.   קיימים תת סוגים של דיסקלקוליה התפתחותית הקשורים לחסכים ספציפיים בחשבון מעבר לחסכים הבסיסיים ב- NUMBER SENSEכאן מחברי המאמר מונים חסכים ספציפיים במגוון תחומים מתמטים – ייצוג כמויות, ייצוג ורבלי של מספרים, ידע של עובדות החשבון, ייצוג חזותי של מספרים, תפיסת רצף המספרים, המערכת העשרונית וייצוג כמויות באצבעות.  לדעתי, לפחות לגבי חלק מהחסכים הספציפיים הללו, מדובר בחסכים בידע מתמטי נרכש באמצעות למידה פורמלית, או מה שמכונה ב – CHC – "יכולת כמותית"   - Gq.        לדעתי, חסכים ב"יכולת הכמותית" נובעים מפגיעה ב – NUMBER SENSE ו/או מפגיעה ביכולות הקוגניטיביות (כלומר ממצבים 1 או 2 המתוארים למעלה), ולכן חסכים אלה הם רק התבטאויות של לקות הלמידה או הדיסקלקוליה ולא סוג נפרד של דיסקלקוליה.

מחברי המאמר ממשיכים ומציינים את הקשיים הבאים, הנגרמים מחוסר האחידות בהגדרה:

א.  חוסר הסכמה בין החוקרים לגבי המשימות שישמשו לצורך אבחנה מבדלת של דיסקלקוליה.  האם להשתמש במשימות בסיסיות הבודקות את ה – NUMBER SENSE, (כמו השוואה מהירה בין ספרה לכמות המיוצגת בנקודות) או להשתמש במשימות מורכבות הכוללות חשיבה מתמטית ו/או הבנת הנקרא (שאלות מילוליות)? 

לדעתי, צריך להשתמש במשימות בסיסיות שבודקות NUMBER SENSE (כפי שיש למשל ב – DYSCALCULA SCREENER  אותו הצגתי בפוסט מה – 9 ביולי) – כדי לאתר את קבוצה מס.1.  צריך להשתמש במשימות מתמטיות מורכבות יותר כחלק מתהליך האיתור של קבוצה מס. 2.

ב.  גם אם תהיה הסכמה לגבי סעיף א', מה תהיה נקודת החתך שמתחתיה יוגדרו הילדים כדיסקלקולים (לצרכי מחקר)?  יש מחקרים שכוללים ילדים שציונם נמוך מאחוזון 10, יש מחקרים שכוללים ילדים שציונם נמוך מאחוזון 35.  כך כוללים המחקרים אוכלוסיה שהיא אולי הטרוגנית מדי.

כל ציון שהוא גבוה מאחוזון 16 יכול להיחשב לציון ממוצע, בהיותו בטווח של סטית תקן אחת ופחות מזה מסביב לממוצע.  לכן, לדעתי, ילדים שמקבלים ציון גבוה מאחוזון 16 במבחן חשבון אינם עומדים בקריטריון הבסיסי לדיסקלקוליה או ללקות למידה (הנמכה מובהקת בהישגים בחשבון).

ג.  האם לדרוש פער בין היכולת הקוגניטיבית הכללית לבין ההישגים
 במתמטיקה? יש מחקרים שכוללים ילדים שאין להם פער כזה, כלומר ילדים המתמודדים עם חסכים קוגניטיבים רחבים ורבים.  לעומתם יש מחקרים שמקפידים לבחור ילדים עם יכולת קוגניטיבית כללית ממוצעת לפחות.

לדעתי, ילד שיש לו לקויות ביכולות קוגניטיביות רבות (עיבוד חזותי, עיבוד שמיעתי, יכולת פלואידית, זיכרון לטווח קצר, מהירות עיבוד, אחסון ושליפה לטווח ארוך, ידע מגובש)  - אבחנתו העיקרית אינה יכולה להיות דיסקלקוליה או לקות למידה.  ולכן חשוב, לדעתי, לבדוק אם קיימות לקויות רחבות כאלה ולא לכלול ילדים כאלה בקבוצת ילדים המוגדרים כדיסקלקולים או לקויי למידה.

Tips for good assessment and report writing - 5


This is the fifth tip written by the assessment  team at the Jerusalem municipality educational psychology services.   The members of the team are:  Rita   Baumgarten, Hanna Brimer, Nadine Caplan , Eynat  Cohen Rahman , Etti Daniel Simon , Uri Dar ,  Michelle Lisses Topaz, Betty Netzer, Ruth Oman Shaked , Adina Sacknovitz , Smadar Sapir Yogev, Anan Srour   and  Dahlia Zayit.
Base your conclusions on an integration of the child's test scores, your clinical impressions and the teacher's reports.  Don't rely only on test scores no matter what, and on the other hand, don't rely only on your clinical impressions.
For example, when you look at the child's fluid ability, consider the child's scores on fluid ability tests,  and also consider your clinical impression of the child's ability to think and conceptualize about abstract ideas, to draw conclusions about things that are not spelled out but only implied in texts, to tell TAT stories that are imaginative, go beyond mere scene descriptions, and are coherent and reasonable, to define concepts and words in an abstract way, to understand complex instructions, to see the main features in RCFT test and to be able to alternate perspectives between parts and whole, to think flexibly and to use feedback effectively, to plan and monitor his work, to refrain from perseverations and automatic responses, etc.  Look at how the teachers describe this child's thinking and understanding, his ability to initiate, plan and self monitor, etc.
If there are significant contradictions between the test scores, the clinical impressions and the teacher's opinion, use your good judgment and good sense to decide whether the child's fluid ability is within normal limits or not, and try to think and hypothesize about the reasons for the contradiction.

Remember:  it's not always possible to reach an unequivocal conclusion.  We are dealing with human beings, so everything does not always 'fit' into place.  When you don't feel confident about your conclusions and have unanswered questions – it's better to say or write your doubts (with suggestions how to resolve them), than to write something you don't feel good with.

כמה דברים שיעזרו לכם לעשות דיאגנוסטיקה טובה יותר - 5


"טיפ" חמישי מסדרה של "עצות" שגובשו בצוות הפיתוח בדיאגנוסטיקה של שפ"ח ירושלים, שמשקפות את ה"אני מאמין" שלנו בנושא דיאגנוסטיקה.  חברי צוות הפיתוח: רות אומן שקד, ריטה באומגרטן, חנה ברימר, אורי דאר, אתי דניאל סימון, דליה זית,  עינת כהן רחמן, מישל ליסס טופז, בטי נצר, סמדר ספיר יוגב, ענאן סרור,  נאדין קפלן ועדינה שכנוביץ.

בססו את מסקנותיכם על שילוב של בסיס הנתונים, ההתרשמות הקלינית ודיווחי המורים.  אל תלכו עם הנתונים "עם הראש בקיר", מצד שני אל תעסקו בדיאגנוסטיקה התרשמותית.

למשל, כדי להסיק על היכולת הפלואידית של הילד, הסתכלו בנתוני המבחנים הבודקים יכולת פלואידית, אך הסתכלו גם על היכולת של הילד לחשוב בצורה מופשטת ולהמשיג, להסיק מסקנות מתוך טקסט על דברים שלא נאמרו בו במפורש, לספר סיפורי TAT שאינם "צמודים" לתמונה ומתארים אותה אלא שיש בהם מן הדמיון, לספר סיפורי TAT בעלי רצף קוהרנטי והגיוני, לתת הגדרות מוכללות למלים ולמושגים, להבין הנחיות מורכבות, לראות את האלמנטים המרכזיים בריי החזותי ולעשות אינטגרציה טובה של החלקים והשלם בו, לחשוב בצורה גמישה ולהפיק תועלת ממשוב, לתכנן את העבודה, לעבוד בצורה מבוקרת, להימנע מפרסברציות ומתגובות אוטומטיות, הסתכלו בדיווחי המורים על יכולת החשיבה וההבנה של הילד, על היכולת שלו לקחת יוזמה ולעבוד בצורה מבוקרת ומתוכננת, וכו'.

במידה ויש סתירה בולטת בין בסיס הנתונים להתרשמות הקלינית ולדיווחי המורים, הפעילו שיקול דעת ושכל ישר בקביעה האם היכולת תקינה או לא, ונסו גם לחשוב על סיבת הפער. 


זיכרו:  לא תמיד ניתן להגיע למסקנה חד משמעית.  מדובר בבני אדם, ולכן לא תמיד הכל  "מסתדר".  כשאנחנו לא בטוחים וכשנשארו לנו שאלות –עדיף לומר/לכתוב את ההתלבטות עם הצעות כיצד ללבן אותה, מאשר לכתוב משהו שאיננו שלמים איתו.

Finding the next Einstein – Dr. Jonathan Wai's blog



This is a most interesting blog written by Dr. Jonathan Wai.  The blog is within Psychology Today, at this address:


Jonathan Wai, Ph.D., is a research scientist at the Duke University Talent Identification Program and a visiting researcher at Case Western Reserve University.

He researches and writes about the development of talent, broadly conceived, and its impact on society.  His interests focus on the role of cognitive abilities, education, and other factors that contribute to the development of expertise in education, occupation, and innovation.  Additionally, he is interested in policy implications of developing (or failing to develop) talent, and connecting his work with the larger global conversation.

The blog features interviews with interesting people, book recommendations, study reviews and more.  Dr. Wai writes in a very interesting way.

Here are two posts I liked:

1.  An interview with Prof.  Douglas Detterman


Prof. Detterman is one of the prominent intelligence scholars
 of our time, and is the founder of the journal "Intelligence".  He's also one of the most generous scholars I've known.  Prof. Detterman is currently writing a book on intelligence.  He sent the book's chapters, in draft, to all those interested provided that the reader sends him comments.  This is how I 
also got to read his fascinating book.  I've learned a lot from 
it. 

In this interview Prof. Detterman discusses social aspects of intelligence.  One of these is  whether cognitive abilities can be improved by training or effort.  From what I gather, Prof. Detterman thinks that beyond a certain point, they can't be much further improved.   "It’s all part of the philosophy of work hard and you’ll be rewarded. That everybody is the same and the only difference is how hard you are willing to work to achieve a particular goal. And if you don’t achieve that goal it’s because you haven’t worked hard enough, not because you’re not capable enough. It would seem to me that a much smarter way of looking at things is that I’m better suited for some things than other things and so I should try to find those things I am suited for and work at those."  This is similar to ROBERT STERNBERG's view (which he terms "Successful Intelligence").

2.  A book recommendation:

Curious: the desire to know and why your future depends on it, by Ian Leslie.




Dr. Wai reviews the book in a way that really makes you want to read it.  I found myself agreeing with most of this book's insights, as represented by Dr. Wai.

מתוך הבלוג אמליץ על שני "פוסטים" שאהבתי במיוחד:

1.  ראיון עם פרופ' דאגלס דטרמן. 


פרופ' דטרמן הוא אחד מחוקרי המשכל הבולטים בדורנו, ומייסד כתב העת INTELLIGENCE.  הוא גם אחד החוקרים הנדיבים ביותר שהכרתי.  פרופ' דטרמן כותב בשנים האחרונות ספר על אינטליגנציה.  הוא שלח את פרקי הספר, במצבם כטיוטא, לכל מי שהתעניין והתחייב לקרוא ולחזור אליו עם הערות/הארות.  כך גם אני קראתי את הספר המרתק, ולמדתי ממנו הרבה מאד.   

בראיון זה דטרמן נוגע בהיבטים חברתיים של המשכל, ובין השאר בשאלה האם ניתן לשפר את היכולות הקוגניטיביות באמצעות אימון או מאמץ.  דטרמן חושב שמעבר לגבול מסוים זה לא ניתן.   האמונה שעבודה קשה תמיד מובילה לתוצאות טובות, המוכרת כחלק מהאתוס האמריקני, מבוססת על ההנחה שכולם שווים וההבדל היחיד בין בני האדם הוא המוכנות להתאמץ.  דטרמן חושב שבגלל שיש הבדלי כשרונות בין אנשים, חשוב שאנשים ידעו למצוא את התחומים בהם הם מוכשרים ולעסוק בהם.  עמדה זו מזכירה את הגישה של ROBERT STERNBERG ("אינטליגנציה מצליחה").

"The Bell Curve" turns 20


  

"The Bell Curve", Herrnstein and Murray's controversial book, was published two decades ago this month.  The book provoked outrage by arguing that it was "highly likely" that the sizable black-white gap in IQ scores is partly genetic.  This unfortunate and unnecessary statement, and others like it  -about IQ differences between ethnic groups, or statements that bordered on eugenics in relation to IQ, as well as deterministic statements (about IQ  being largely hereditary) – caused many good scholars to abandon this field.

עקומת הפעמון" בן 20"



"עקומת הפעמון", ספרם השנוי במחלוקת של Herrnstein and Murray אודות האינטליגנציה, יצא לאור החודש לפני 20 שנה בארה"ב.  ספר זה עורר סערה כאשר טען שיש "סבירות גבוהה" שהפער ב – IQ בין שחורים ללבנים הוא גנטי בחלקו.  אמירה אומללה ומיותרת זו  ודומות לה, למשל על הבדלים במשכל בין קבוצות אתניות, או אמירות שנגעו ב"השבחת הגזע" בהקשר של IQ, כמו גם אמירות דטרמיניסטיות על IQ (על כך שהוא נקבע בצורה משמעותית על ידי התורשה, על כך שהוא לא ניתן לשינוי משמעותי) -  גרמו לדעתי לחוקרים רבים וטובים לברוח מעיסוק בתחום.  זו אולי אחת הסיבות לכך שבישראל אין אקדמאים שעוסקים באינטליגנציה אנושית.

Detecting signs of Alzheimer's disease through written language analysis – and what does it have to do with Iris Murdoch?

Understanding the paper in depth requires broader linguistic knowledge than I have, but it's main ideas are quite interesting.

Apparently, it's possible to track signs of cognitive impairment/ decline (as occurs in Alzheimer's disease, for example) by written or oral language analysis that measures aspects like syntactic complexity of the sentences, idea density, use of  referring expressions and discourse coherence.

The early linguistic signs of  Alzheimer's disease  are manifested in semantic declines.  Semantically "empty" speech characterized by overuse of pronouns has been noted as one of the distinctive features of the disorder, as well as semantic deficits affecting one's ability to determine semantic relatedness between concepts.  Cognitive declines in Alzheimer's disease   are also associated with poorer performance in tasks related to working memory, especially in the more advanced stages of the disease.  Syntactic complexity in healthy people is associated with working memory demands and is predictive of the processing time in sentence comprehension.

Iris Murdoch is an Irish philosopher and writer, and is certainly one of my favorite writers.  Murdoch lived in 1919-1999, and published 25 books and philosophy works.  Her first book was published in 1954 and her last – four years before her death – in 1995.  I especially like her book "The nice and the Good" and warmly recommend it.





 In 1997 Iris Murdoch was diagnosed with Alzheimer's disease.  This diagnosis was confirmed after her death by pathology results.

Iris Murdoch's books are especially suitable for linguistic analysis because she resisted any editing of her writing prior to publication, so it's safe to assume that her books are representative of her actual language production.

There are several manual and computerized methods for semantic and syntactic text analysis.  For example, it's possible to count the average number of words per sentence, the average number of clauses per sentence, the proportion of times ten most common words in each text are repeated within the space of five words, etc.

This study used computerized methods to analyze the syntactic complexity in four Iris Murdoch books:  her first book ("Under the Net"), published in 1954, "The Sea, The Sea", published in 1978, "The Green Knight" , published in 1994 and "Jackson's Dilemma", published in 1995.  The researchers analyzed 20 randomly selected passages (that did not contain dialogues) from each book. 

A clear pattern of decline in sentence length, number of clauses per sentence and other measures of syntactic complexity was found across the four books.


Syntactic complexity declines are present in healthy elderly people (particularly older than 74).  Iris Murdoch was 75 years old when "Jackson's Dilemma" was published and 74 years old when "The Green Knight" was published.  Thus it's difficult to tease out the effect   sizes of normal aging and Alzheimer's disease (since there are no age and education based norms for written output).
