חמש הסכמות בין מומחים לגבי הערכת לקות למידה ספציפית
CRITICAL ISSUES IN RESPONSE-TO INTERVENTION,
COMPREHENSIVE EVALUATION, AND
SPECIFIC LEARNING DISABILITIES IDENTIFICATION AND INTERVENTION: AN EXPERT WHITE
PAPER CONSENSUS
מסמך זה מ – 2010 מסכם באופן תמציתי סקר שנערך בין 58 מומחים אמריקנים
בתחום לקות למידה ספציפית, הערכה קוגניטיבית או נוירו פסיכולוגית של לקות למידה
ספציפית, התערבות חינוכית בלקות למידה ספציפית ונושאים משפטיים הקשורים ללקות
למידה ספציפית. לא השתתפו בסקר חוקרים
שדגלו בעיקר במודל הפערים או במודל תגובה להתערבות. מטרת הסקר היתה להשפיע על קביעת מדיניות
בארה"ב בנושא לקות
למידה ספציפית.
אביא כאן את תוצאות הסקר, עם תוספות שלי בצבע כתום.
כדי להבין טוב יותר את ההסכמות צריך לדעת מה זה
IDEA 2004, מהו מודל הפערים, מהו מודל תגובה להתערבות, מהי גישתה של FLANAGAN להגדרת לקות
למידה. כל אלה מוסברים במצגת "לקות
למידה – סיפורה של הגדרה" שניתן לקרוא בלחיצה כאן.
המומחים הסכימו בינהם על חמישה דברים:
1.
מומלץ לשמר את הגדרת לקות למידה ספציפית הקיימת (IDEA 2004). מומלץ להקפיד יותר בתהליך הדיאגנוסטי על עמידה
בדרישות ההגדרה. 82% מהמומחים התנגדו לכך
שההגדרה תורחב ותכלול כל ילד שמציג הישגים נמוכים. כלומר, הישג נמוך בלבד אינו יכול להיות,
לדעת מומחים אלה, סמן מתאים ללקות למידה ספציפית.
2.
הן מודל הפערים והן מודל תגובה להתערבות אינם מספיקים לבדם לאבחנת
לקות למידה ספציפית.
3.
הגישה הגיונית ביותר מבחינה אמפירית וקלינית לזיהוי לקות למידה ספציפית היא
גישה שמזהה חוזקות וחולשות בתהליכי עיבוד פסיכולוגים, וחסכים בהישג שעולים בקנה
אחד עם חולשות אלה.
הסכמה
זו תומכת בגישתה של FLANAGAN להגדרת לקות למידה, בהתאם לתאורית CHC. במונחים של CHC הכוונה היא לזיהוי
יכולות קוגניטיביות חזקות/תקינות וחלשות אצל הילד (מבין היכולות הבאות: יכולת פלואידית, ידע מגובש, זיכרון לטווח קצר,
מהירות עיבוד, אחסון ושליפה לטווח ארוך, עיבוד חזותי, עיבוד שמיעתי), זיהוי הנמכות
בהישגים בקריאה, כתיבה וחשבון, וקישור בין היכולות הקוגניטיביות הנמוכות להנמכות
שזוהו בהישגים.
4.
אפשר להשתמש בגישת תגובה להתערבות כדי למנוע בעיות בלמידה או עיכובים
בלמידה, אבל בשביל לאבחן לקות למידה ספציפית נדרשת הערכה מקיפה. ילדים עם לקות למידה ספציפית זקוקים להתערבויות
שמתייחסות באופן ספציפי לצרכי הלמידה שלהם (המבוססים על החולשות והחוזקות בתהליכים הקוגניטיביים
שלהם) ולא רק התערבות אינטנסיבית יותר (כפי שמציע מודל תגובה להתערבות).
מרבית המומחים חשבו שהן מודל תגובה
להתערבות והן הערכה מקיפה של תהליכים פסיכולוגים חשובים בתהליך ההגדרה וההתערבות
בלקות למידה ספציפית.
הובעה דאגה, שמודל תגובה להתערבות עלול
להפוך, כמו מודל הפערים, למודל של "המתנה לכישלון", כאשר במקום לבנות
התערבות שמתאימה ספציפית לקשיים של הילד, מגבירים את האינטנסיביות של ההתערבות
לאחר שהילד לא הגיב לה. המומחים מציעים
להגביר את הגמישות בשימוש במודל תגובה להתערבות.
כשילד לא מגיב להתערבות ראשונית אולי עדיף לערוך מייד הערכה מקיפה של
תהליכים פסיכולוגים כדי לבדוק האם הסיבה לאי התגובה היתה לקות למידה ואם כן –
כדי לתכנן התערבות ספציפית שתתאים לו – ולא להסתפק בהגברת האינטנסיביות של
ההתערבות.
5.
הערכה של תהליכים קוגניטיבים ונוירו פסיכולוגים צריכה לשמש לא רק לזיהוי של
לקות למידה ספציפית אלא גם לתהליך ההתערבות.
נדרש מחקר אמפירי רב יותר על הקשרים בין תהליכים קוגניטיבים לבין דרכי
התערבות. (כלומר צריך לחקור יותר אילו פגיעות בקריאה, כתיבה וחשבון
נגרמות מאילו הנמכות ביכולות קוגניטיביות, עד כמה אימון של יכולות קוגניטיביות
מסוימות יכול לשפר הישגים בקריאה, כתיבה וחשבון, ואיזה סוג של אימון עוזר).

וכיצד היכולות הקוגניטיביות משפיעות על מאפייני האישיות. למשל, ילד שמתקשה לעשות אינטגרציה של פרטים
לשלם בגירויים חזותיים מורכבים, ומתקשה גם לעשות אינטגרציה בין הדמויות השונות
ובין הדמויות והאובייקטים ב – TAT וליצור מהם סיפור אחד מורכב. או נער שמתקשה להבחין בבירור בין נקודות המבט
של עצמו ושל אחרים, שמתקשה לראות את עצמו ואת האחרים כישויות פסיכולוגיות בעלות
מניעים וחוויות מורכבים, מתאר את הדמויות בסיפורי TAT בעיקר באמצעות מאפיינים חיצוניים/נצפים, ומתקשה
להבין את הנקרא כשהוא נדרש לייחס כוונות לדמויות בטקסטים שהוא קורא.

Mathematics and cognitive abilities part 1: the development of arithmetic skills.
The first of three presentations about
mathematics and cognitive abilities is found here. This presentation deals with the development
of arithmetic skills, especially those we need to know in order to understand
learning disabilities manifested in math and dyscalculia.
What will the other presentations be
about? (It'll take me a while longer to get them in a publishable state).
Part 2 will deal with cognitive abilities (fluid
ability, processing speed, short term memory, long term storage and retrieval,
visual processing, auditory processing, comprehension knowledge) and their
relations with math.
Part 3 will deal
with learning disabilities manifested in math and dyscalculia.

A classic paper about the development of arithmetic abilities
Those of you who visit here once in a while, may
have noticed that I write a lot about cognitive abilities and math. The reason is that I've decided to upgrade and
reorganize an old presentation about this subject. That is how I re-read Butterworth's paper:

This is an outstanding paper, in my
view. It's written in a clear and an eloquent way, and it presents the
main issues in the development of arithmetic abilities that are relevant
to understanding learning disabilities manifested in math and
dyscalculia. I strongly recommend it to
those interested in this subject. At the
end of this paper (which I won't review here, because it's main points will be
included in the presentation I'm preparing and hoping to publish soon) - Butterworth
writes about dyscalculia and defines it according to his point of view (as a
difficulty with number sense).

Tips for good assessment and report writing - 6
This is the sixth tip written by the
assessment team at the Jerusalem

Start
"thinking abilities" - not
"thinking tests"
Don't think: "I'll assess the child with
the Wechsler test, TAT, DST" etc.
Tests
are means, not ends.
Try thinking: "which cognitive
abilities and skills should I assess in depth with this child, in order to
answer the assessment question? Which lowered
abilities or skills might cause the specific difficulties this child has? How do I assess these abilities or
skills?"
Our assumption is that lowered cognitive
abilities may cause difficulties in reading, writing and math, and may also affect
the child's social and emotional functioning (exactly as emotional difficulties the
child has may also affect his cognitive performance). In order to work in light of this assumption,
you should know about the relations between cognitive abilities (fluid ability,
processing speed, short term memory, long term storage and retrieval, visual
processing, auditory processing, comprehension-knowledge), achievement (reading,
writing, math) and social-emotional wellbeing.
You should know how a specific low ability may affect the child's reading, writing, math and social
and emotional wellbeing.
These relations are discussed in the
presentation series "intelligence and cognitive abilities" found in
the right hand column, and in some of the posts in this blog.

אל תחשבו: "אבדוק את הילד בוכסלר, TAT, מבחן מסלול וכו'".
המבחנים הם אמצעי, ולא מטרה.
נסו לחשוב: "אילו יכולות קוגניטיביות ואילו כישורים עלי
לבדוק לעומק אצל הילד, כדי להשיב על שאלת האבחון?
אילו יכולות פגועות או כישורים
פגועים עלולים להיות הגורמים לקשיים הספציפיים שיש לילד שלפני? כיצד אבדוק את היכולות או הכישורים הללו?"
אנחנו יוצאים מנקודת
הנחה, שקשיים שיש לילד ביכולות הקוגניטיביות הם חלק מהגורמים לקשיים שיש לו בתפקוד
הלימודי, והם משפיעים גם על התפקוד החברתי והרגשי שלו (בדיוק כפי שקשיים שיש לילד
במישור הרגשי, משפיעים גם על התפקודים הקוגניטיבים שלו).
כדי לעבוד לאור הנחה זו, עליכם להכיר את הקשר
בין היכולות הקוגניטיביות (יכולת פלואידית, מהירות עיבוד, זיכרון לטווח קצר, אחסון
ושליפה לטווח ארוך, עיבוד חזותי, עיבוד שמיעתי, ידע מגובש) לבין סוגים שונים של

Dyscalculia – is it chronic? Findings from an Israeli long term study

Prof. Ruth Shalev, Prof. Varda Gross-Tsur and Dr. Orly Manor followed, for six years, in
a wide scope study, a group of children diagnosed with dyscalculia.
How were
the children recruited?
In fourth grade, about 3000 children studying
in Jerusalem schools took a group arithmetic test. 550 out of the 600 children whose scores were
in the low 20% took, in fifth grade, an individually administered arithmetic
test. 140 children out of this group
scored in or below the 5th percentile in the arithmetic test and had
a WISC-R IQ score above 80. This group
was diagnosed with dyscalculia. The
reading and writing skills of these140 children were assessed, and they were given other cognitive tests as
well (which I'll not go into here for sake of brevity).
After three years, when they were in 8th
grade, 123 children out of this group took a math test and a reading test
again. The math test scores of 95% of
the children were in the 25th percentile or below. 47% of these children were re-diagnosed with
dyscalculia, having scored in the 5th percentile or below.
After three more years, when they were in 11th grade,
104 of these children took math, reading and writing tests again and were
compared to a control group.
What were the findings?
The authors emphasize the performance of
the dyscalculia group, but I think it's worthwhile to look also at the control
group's performance.
Let's begin with four
examples:
·
51% of the 104 11th
grade students identified in 5th grade with dyscalculia were not able to solve 8x7, compared to 17% of
the control group.
·
71% of the 104
students were not able to solve 24x37, compared to 27% of the control group.
·
49% of the 104
students were not able to solve 45/3 compared to 15% of the control group.
·
63% of the 104
students were not able to solve 5/9+2/9, compared to 17% of the control group.
And in general:
40% of the 104 students scored in or below the 5th
percentile, and were re- diagnosed with
dyscalculia.
The authors point out that the scores of most remaining 60% of
children was still "low" – in and under the 25th
percentile. Since every score which is
higher than the 16th percentile is within one standard deviation
below the mean, we can regard such scores as normal performance (even if not
high). The authors don't indicate what
percentage of the 104 children had a score higher than the 16th
percentile.
Which 5th grade measures were related to
dyscalculia in 11th grade?
The 5th grade general IQ score, calculated without the arithmetic
subtest, was in average 6 points lower in the 104 student group than in the
control group. The 104 student group
also had more inattention and writing difficulties than the control group.
Which 5th grade
measures were NOT related to dyscalculia in 11th grade?
Reading, word learning, fluency tests, face recognition and
performance in RCFT test were not related to dyscalculia in 11th
grade.
Educational interventions, socioeconomic status, parental
education, gender and family history of learning difficulties were not related
to dyscalculia in 11th grade.
What do we learn from all
this?
Apparently, dyscalculia as defined here (a score in or below the 5th
percentile in an arithmetic test and an IQ score within normal limits) is
chronic in 40% of the cases. Had we
defined dyscalculia as a score in or below the 16th percentile in an arithmetic test
and an IQ score within normal limits, probably a higher percentage of the 104 student group would have been diagnosed with chronic dyscalculia.
Nevertheless, there were some children in this study who made
progress and moved from performance in or below the 5th percentile
in 5th grade, to performance of above the 16th percentile in 11th
grade. It's unclear what caused this
improvement. This is a question worth
studying. We can also hope, that early assessment, much earlier than 5th grade, maybe even in
preschool, and preventive intervention, will make it possible to prevent the
development of dyscalculia in at least some of the children.

ורדה גרוס ודר' אורלי מנור עקבו במשך שש שנים, במחקר מעורר כבוד בהיקפו, אחר קבוצת
ילדים ירושלמים שאותרו כדיסקלקולים.
כיצד אותרו ילדים אלה?
בכיתה ד', עברו כ –
3000 ילדים ירושלמים מבחן קבוצתי בחשבון. 550 מתוך 600 הילדים שציוניהם היו ב – 20%
התחתונים עברו בכיתה ה' מבחן חשבון אישי. מתוך
קבוצה זו אותרו 140 ילדים שציונם במבחן זה
היה באחוזון 5 ומטה, וציונם במבחן WISCR היה מעל 80.
קבוצה זו, שחבריה הוגדרו כדיסקלקולים, עברה הערכה בקריאה ובכתיבה, וכן
מבחנים קוגניטיבים נוספים שלא אפרט כאן כדי לא להלאות.
כעבור שלוש שנים, בהיותם
בכיתה ח', עברו 123 מילדי קבוצה זו שוב מבחן במתמטיקה וקריאה. הציונים בחשבון של 95% מהם היו באחוזון 25 ומטה. 47% מהם
הוגדרו מחדש כדיסקלקולים מכיוון שציונם היה באחוזון 5 ומטה.
כעבור שלוש שנים
נוספות, בהיותם בכיתה יא', עברו 104 מילדי קבוצה זו שוב מבחנים בחשבון, קריאה
וכתיבה, והשוו לקבוצת ביקורת.
מה נמצא?
החוקרות מדגישות את
הנתונים של הקבוצה הדיסקלקולית, אך אני חושבת שראוי להתבונן באותה מידה בביצועים
של קבוצת הביקורת.
נתחיל בארבע דוגמאות:
·
51% מ-104 תלמידי יא' שאובחנו בכיתה ה' כדיסקלקולים, לא הצליחו לפתור
את התרגיל 7X8,
לעומת 17% מקבוצת הביקורת.
·

· הציון של 40% מ-104 התלמידים במבחן המתמטיקה היה באחוזון 5 ומטה, ולכן הם אובחנו מחדש כדיסקלקולים.
· החוקרות מציינות שהציון של מרבית הילדים מה – 60% הנותרים היה עדיין "נמוך" – באחוזון 25 ומטה. מכיוון שכל ציון שהוא גבוה מאחוזון 16 נמצא בטווח של סטית תקן אחת ופחות מזה מתחת לממוצע, ניתן לראות ציון כזה כתפקוד תקין (אם כי לא גבוה). החוקרות לא מציינות איזה אחוז מ – 104 הילדים קיבל ציון באחוזון 16 ומעלה.

· ציון ה – IQ הכללי מכיתה ה', שחושב בלי תת מבחן חשבון, היה נמוך בשש נקודות בממוצע אצל 104 התלמידים, לעומת קבוצת הביקורת. קבוצת ה – 104 גילו, לעומת קבוצת הביקורת, גם קשיים בקשב ובכתיבה.
אילו מדדים שנמדדו בכיתה ה' לא היו קשורים לדיסקלקוליה בכיתה יא'?
· קריאה, למידת מלים, מבחני שטף, זיהוי פנים, מבחן RCFT, לא היו קשורים לדיסקוקליה בכיתה יא'.
· גם קבלת עזרה בחשבון, מצב סוציו אקונומי,השכלת ההורים, מין הילד, והיסטוריה משפחתית של קשיי למידה לא היו קשורים לדיסקלקוליה בכיתה יא'.
מה אנחנו לומדים מכל זה?
· נראה שדיסקלקוליה, כפי שהיא מוגדרת כאן (ציון במבחן חשבון באחוזון 5 ומטה ומשכל תקין) היא מצב שאצל 40% מהלוקים בו הוא כרוני. אם נגדיר דיסקלקוליה כציון במבחן חשבון באחוזון 16 ומטה ומשכל תקין, ככל הנראה אחוז גבוה מ – 40 מקבוצת ה - 104 יוגדרו כמתמודדים עם דיסקלקוליה כרונית.
· בכל זאת היו ככל הנראה מספר ילדים, שהתקדמו מתפקוד באחוזון 5 ומטה בכיתה ה', לתפקוד באחוזון של מעל 16 בכיתה יא'. זו בעיני התקדמות משמעותית, גם אם הם לא הגיעו לתפקוד ברמה של מעל אחוזון 25. לא ברור מה עזר לילדים אלה להתקדם. זו שאלה שחשוב בעיני להשיב עליה. ניתן גם לקוות, שבעזרת איתור מוקדם, הרבה הרבה לפני כיתה ה', ואולי עוד בגן, והתערבות מונעת, ניתן יהיה למנוע את התפתחות הדיסקלקוליה לפחות אצל חלק מהילדים.

The need for a unitary definition of dyscalculia
Those of you who've read my presentation
– "Learning disability – the story of a definition", saw the
difficulties and confusion caused to children and to research by the lack of
agreement among experts about almost each of the basic features of learning
disability.
This paper strives to get at a unified
definition of Developmental Dyscalculia (DD).

The
paper distinguishes between three approaches to dyscalculia, which I'll rephrase
in light of my own viewpoint and interpretation:
1.
DD is related to basic deficiencies in number sense. Here we refer to a group of children who have
poor grasp of number magnitude. These children are slower to determine, for instance, whether the
amount of dots in an array is equal to a specific numeral (numerals and quantities
below 9). Butterworth argues that this specific difficulty indicates deficiency
in what he calls "the number module".
2. DD subtypes are caused by deficits in various cognitive
processes. Deficiencies in verbal
working memory, semantic memory, visuospatial processing or fluid ability
affect mathematic functioning. I prefer
to name this group "learning disabled" rather than "DD". This is because the disabilities this group
has in cognitive abilities (visuospatial processing, short term memory,
comprehension-knowledge etc.) usually affect not only math but also reading, writing and
reading comprehension. Each of these
children is learning disabled in a different way (according to the specific
affected cognitive ability) and so his performance in math (and also reading,
writing and reading comprehension) will be – I think- different.
3. DD subtypes are related to
specific deficiencies in math beyond the basic deficiencies in number sense. Here the authors list specific deficiencies
in various math areas – magnitude representation, verbal representation of
numbers, knowledge of arithmetic facts, visual representation of numbers,
ordinality, the base 10 system, finger representations of numbers. I believe, that at least some of these
specific deficits are deficits in acquired math knowledge (knowledge that is
learned in school), or in CHC terminology – "quantitative ability" –
Gq. I think that deficiencies in
"quantitative ability" might be caused by poor number sense and/or
disabilities in cognitive abilities (meaning, situations that are described
under 1 and 2 above). That's why deficiencies in Gq are only manifestations of learning
disability or dyscalculia and not a separate kind of dyscalculia.
The authors go on to point out the following problems caused by the
lack of a unitary definition:
A. Disagreement among experts about which tasks should
be
used to make a differential diagnosis of DD. Should we use basic tasks measuring number sense
(like quickly comparing a numeral to an array of dots) or should we use complex
tasks that include math reasoning and/or reading

B. Even if agreement is reached about problem A, what
should be the cutoff point under which children will be identified as DD (for
research purposes)? Some studies
include children whose scores are lower than the 10th
percentile. Other studies include
children whose scores are lower than the 35th percentile. Thus studies include a population which might
be too heterogenous.
A score higher than the 16th percentile can be
considered to be an average score, being
within one standard deviation below the mean.
So I believe that children with scores above the 16th
percentile in math tests do not satisfy the basic criterion for dyscalculia or
learning disability (namely, significant underachievement in math).
C. Should we require a discrepancy between the general cognitive ability
and math achievement? Some
studies include children with no such discrepancy – children who struggle with
broad cognitive deficits. Other studies choose children with at least average
general cognitive ability.
I think, that the main diagnosis of a child who has disabilities in many cognitive abilities (visuospatial processing, auditory processing, fluid ability, short term memory, processing speed, long term storage and retrieval, comprehension knowledge) is not dyscalculia or learning disability. That's why it's important, in my opinion, not to include children with broad cognitive deficits in groups of children meant to be with dyscalculia or learning disability.
