אינטליגנציה - כל מה שחשוב

זהו ספר דק וקטן מימדים (115 עמודים, ללא הנספחים והמלצות לקריאה)
המציג את נושא המשכל בצורה רחבה ומעניינת.
הספר כתוב לקהל הרחב ומתאים גם לאנשי מקצוע. המחבר ד"ר סטיוארט ריצ'י הוא חוקר במחלקה
לפסיכולוגיה באוניברסיטת אדינבורו. הוא
חוקר את ההתפתחות של האינטליגנציה והשתנותה במהלך החיים.
נושא המשכל הוא רגיש מאין כמותו.
קל לנו לקבל הבדלים בין אנשים ביכולת הספורטיבית, למשל, אבל קשה לנו כחברה

והנה כמה עובדות כאלה:
משכל קשור לתוחלת חיים. אנשים בעלי רמת משכל גבוהה יותר חיים יותר, פשוטו כמשמעו. במחקר שנערך בשבדיה והתפרסם ב – 2008 נבדקה רמת
המשכל של יותר ממיליון גברים. גברים שציון
המשכל שלהם נמצא בקבוצה הנמוכה מבין תשע היו בעלי סיכון גבוה (יותר מפי 3) למות
בעשרים השנים שלאחר מכן מאשר גברים שציון המשכל שלהם נמצא בקבוצה הגבוהה מבין תשע. למה זה כך?
ייתכן שאנשים בעלי רמת משכל גבוהה יותר מודעים יותר להתנהגויות מקדמות
בריאות כמו בדיקות תקופתיות, פעילות גופנית, תזונה נכונה. אנשים בעלי רמת משכל גבוהה יותר הם בעלי סיכוי
גבוה יותר ליהנות ממיצב סוציואקונומי גבוה יותר, המאפשר להם לממן ביטוחי בריאות
טובים יותר, לפנות זמן רב יותר לפעילות גופנית ולרכוש מזון בריא שלעתים הוא גם יקר
יותר. אנשים בעלי רמת משכל גבוהה יותר הם
בעלי סיכוי גבוה יותר לעסוק בעבודות שיש בהן פחות סיכון בריאותי (פחות סיכון
לתאונות עבודה, פחות סיכון לחשיפה לחומרים מסוכנים).
במחקר השבדי התברר שמשכל נמוך היה גורם סיכון לכך שאדם יירצח בעשורים
הבאים. משכל נמוך קשור גם לסיכון גבוה
יותר של מוות בתאונות. ייתכן שאנשים בעלי
משכל גבוה מצליחים יותר להבחין בסיכונים ולהגיב להם או להימנע מהם.
הקשר בין רמת המשכל לבריאות הוא קשר של מתאם ולא קשר סיבתי. במידה מסוימת רמת המשכל מושפעת מבעיות בריאות ומבעיות
תזונה. ייתכן שאנשים שנולדו למיצב
סוציואקונומי נמוך מקבלים תזונה פחות טובה שפוגעת להם ברמת המשכל.
חמישים אחוז מההבדלים בין אנשים
ברמת המשכל הכללית הם גנטיים (צריך לזכור שזה נתון ממוצע, שנכון לקבוצות גדולות של אנשים ולא תמיד נכון
לאדם ספציפי). אלה חדשות
"טובות" ו"רעות" כאחד:
מחד זה אומר שרמת המשכל הכללית מושפעת מאד מגנטיקה, ואלה חדשות רעות עבור
אנשים שמזלם הגנטי לא שפר עליהם בהיבט של המשכל.
מאידך חמישים אחוז מההבדלים בין אנשים ברמת המשכל הכללית הם לא גנטיים. משמעות הדבר היא שיש לסביבה בה האדם גדל
ולחינוך שהוא מקבל השפעה על המשכל.
בשנת 1960, הממשלה הנורבגית החליטה להוסיף עוד שנתיים של לימודים לחוק
חינוך חובה. החוק הזה יושם באופן מדורג
באזורים שונים בנורבגיה. כל הגברים
הנורבגים מגויסים לשירות חובה בצבא. כאשר
הוא מתגייס, כל גבר נורבגי עובר מבחן משכל.
בשנת 2012 השוו חוקרים בין ציוני המשכל של מתגייסים שלמדו שנתיים נוספות
לבין ציוני המשכל של מתגייסים שלא למדו שנתיים נוספות, מכיוון שהחוק טרם יושם באזור
מגוריהם. החוקרים מצאו שכל שנה נוספת של לימודים הוסיפה
3.7 נקודות לציון המשכל הכללי.
קיים מתאם חזק בין ציון המשכל לבין ציוני בחינות בבית הספר, ולבין
שנות חינוך (האם אדם ינשור מהתיכון והאם ימשיך לאוניברסיטה). החוקר IAN DEARY, אחד מחוקרי המשכל
הבולטים בעולם, בדק במחקר שהתפרסם ב - 2007 את המשכל של 13,000 ילדים בגיל 11, ואת
ההישגים של אותם ילדים בגיל 16 במבחני הישג סטנדרטים באנגליה ובווילס. המתאם
בין ציון המשכל בגיל 11 לבין הציון במבחני ההישג בגיל 16 היה 0.81. זהו מתאם גבוה מאד. משמעותו היא שמבחני ההישג בדקו משהו מאד דומה
למה שמבחני המשכל בדקו חמש שנים קודם לכן.
אמרנו קודם, שהמשכל מושפע מאד מגנטיקה. באופן מוזר, ההשפעה הגנטית על המשכל היא נמוכה יחסית אצל ילדים (השפעה
גנטית של כ – 20 אחוזים) וחזקה יותר אצל
מבוגרים (השפעה גנטית של עד 80 אחוזים).
איך מסבירים את זה? נראה, שילדים
פתוחים יותר להשפעות של הסביבה על המשכל, בין השאר באמצעות בית הספר. ייתכן שכמבוגרים, אנו נוטים לבחור סביבות עבודה
שתתאמנה יותר ויותר לתחומים הקוגניטיבים בהם אנו חזקים וכך אנו מאמנים פחות ומאתגרים
פחות את התחומים הקוגניטיבים בהם אנו חלשים.
עד כמה ההורות משפיעה על המשכל? את זה ניתן ללמוד בין השאר ממחקרי תאומים.
סביבה משותפת היא כל הדברים שעשויים להפוך זוג תאומים לדומים יותר זה
לזה: סגנון הורות, מספר הספרים בבית,
המיצב הסוציואקונומי של השכונה ועוד.
סביבה לא משותפת היא כל הדברים שהופכים תאומים לשונים יותר זה מזה: מורים שונים שיש לכל אחד מהם, קבוצת חברים שונה
שיש לכל אחד מהם, חוויות שונות שכל אחד מהם חווה.
מחקרי תאומים מראים שלסביבה
המשותפת יש השפעה קטנה מאד על המשכל. כאשר
בודקים את רמת המשכל הכללית של תאומים בבגרות, כמעט כל השונות מוסברת על ידי
שילובים של גנטיקה וסביבה לא משותפת. חוץ
מבמקרים של התעללות או הזנחה, לסגנון ההורות אין השפעה חזקה על האינטליגנציה של ילדים
בטווח הארוך.
חוקר המשכל רוברט פלומין Robert Plomin הצביע על כך שככל שאתה
מבין יותר בגנטיקה של המשכל, כך אתה יכול פשוט להירגע ולאהוב את הילדים שלך כפי
שהם וכפי שהם יתפתחו להיות, ואתה יכול לזנוח את הרעיון שילדים נולדים כלוח חלק
שעליך לעצב בעצמך. זה מסיר חלק מהנטל
ומהחרדה מההורות.
בחברה שיש בה פערים סוציואקונומים גדולים, התורשתיות של רמת המשכל
הכללית נוטה להיות גבוהה יותר במשפחות ממיצב סוציואקונומי גבוה מאשר במשפחות ממיצב
סוציואקונומי נמוך. זאת אולי מכיוון שילדים
ממשפחות ממיצב סוציואקונומי גבוה ממצים את כל מה שהסביבה המעשירה יכולה להעניק להם,

ילדים ממשפחות ממיצב סוציואקנומי נמוך, לעומתם, לא תמיד ממצים את מה
שהסביבה יכולה להעניק להם, ולכן הבדלים ביניהם ברמת המשכל יכולים להיגרם יותר בשל
הבדלים בהזדמנויות העשרה והתפתחות שהסביבה מעניקה להם.
ריצ'י
כותב על ניסוי מחשבה שמציע הפסיכולוג סטיבן פינקר. פינקר מציג עולם דמיוני בו המשכל נקבע לגמרי על
ידי הסביבה, ואין עליו השפעה ביולוגית/גנטית כלל.
אין סיבה, לדעת פינקר, לחשוב שעולם כזה יהיה מקום טוב יותר. בעולם כזה, הסביבה הסוציואקונומית תגביל את
ההתפתחות האינטלקטואלית של כל ילד שנולד למשפחה עניה לנצח. בעולם כזה הממשלה תוכל לקבוע מדיניות חברתית
באמצעותה היא תדאג לכך שאנשים מסוימים יחיו בסביבה מקפחת/לא מטפחת וכך תגרום לכך שאותה
קבוצה של אנשים תהיה בעלת רמת משכל נמוכה.
זה נשמע כמו תסריט בלהות. מסתבר
שיש יתרונות לכך שהמשכל מושפע מגנטיקה.

Considerations at the base of DSM5 learning disorder criteria - Part B
Tannock,

This is
part B of a post discussing a document by Prof. Rosemary Tannock, a member of the DSM-5 Work-Group for ADHD and
Disruptive Behavior Disorders and a Liaison member of the Neurodevelopmental
Disorders Work Group to advise on Learning Disabilities. I'll continue
to present Prof. Tannock's views in
black or orange and my thoughts
in green.
In this
part, Prof. Tannock first tries to explain why the work group abandoned the
IQ-achievement discrepancy criterion for learning disabilities (LD) in
DSM5. Then she tries to explain why the
work group also waived the possibility to include cognitive processing deficits as a criteria for LD.
According to the IQ –
achievement discrepancy model, an LD child is a child with poor achievement in
reading/writing/mathematics and a full scale IQ that is average or above
average. Thus this child has a
discrepancy between his achievement level in reading/writing/math and his IQ
score. The child is learning disabled if
we can't explain his low achievement better with one of the excluding factors
(a sensory problem like poor vision, intellectual disability, emotional or
behavioral problems, lack of motivation, insufficient instruction, poor English
proficiency and so on). If one of these
excluding factors explains the child's poor achievement better, the main cause of
his difficulties is probably not learning disability.
According to the
IQ-achievement discrepancy model, LD is a situation in which the child has specific
cognitive deficits that lower his reading/writing/math achievement, but due to
his average intellectual ability, the child has a good chance to withstand the disability or "bypass" it. Such a child may have trouble with reading
comprehension because he reads slowly and inaccurately, but may be able to
understand the same text well if it is read to him.
According to the
IQ-achievement discrepancy model, LD is not a situation in which a child
has many poor cognitive abilities. Such
broad deficits usually lower the general intelligence score. A child with many poor cognitive abilities
will find it hard to "bypass" his difficulties. Such a child may have trouble with reading
comprehension because he reads slowly and inaccurately, but may also find it
difficult to understand the same text if it is read to him. That's because his general and lexical knowledge,
abstract thinking and deduction abilities may also be poor and lower his
listening comprehension.
Prof.
Tannock writes:
"The logic behind the IQ-discrepancy
definition is that the cause of the learning difficulties would differ between
those with and without IQ-Achievement discrepancy".
I think the same cognitive
disabilities can cause problems in acquiring reading/writing/mathematics both
with people with (at least) average IQ (people with discrepancy) and with
people with an IQ score lower than average (people without a discrepancy). The difference between the groups is that
people without a discrepancy have more cognitive deficiencies/disabilities
than people with a discrepancy. The
reason for that is simple: the general
IQ score is comprised of a person's scores in the various cognitive
abilities. The more poor abilities a
person has, the lower is his IQ score. This
quantitative difference (a child having many poor abilities) makes a
qualitative difference in the ability to learn and problem solve.
"Thus, we sought evidence to support
or refute the notion that individuals with learning difficulties with and
without an IQ-achievement discrepancy differ in clinically meaningful ways…"
"..numerous
studies (e.g., Fletcher et al., 2005; Francis et al., 2005; Siegel, 1992) and meta-analyses (Hoskyn,
2000; Maehler &
Schuchardt, 2009; Stuebing et al., 2002) have been conducted to test for
differences between discrepant and non-discrepant groups of children with LD in
terms of cognitive processes that contribute to learning. Findings are
consistent: the two groups do not differ
in their cognitive processing skills".
I looked at some of the
papers cited here. The Maehler &
Schuchardt, 2009 study, for instance, is not a
"meta-analysis". This research
compared between three groups of children in grades 2 to 4. Each group had 27 children (a minimal group
size for acceptable statistical results is 30, if I'm not mistaken).
In the control group the
children did not have reading difficulties. In the other two groups the
children had significant reading difficulties.
The children in one of the
reading difficulty groups had an "IQ-achievement discrepancy". The children in the other reading difficulty
group did not have such a "discrepancy".
The authors found that the
control group children's working memory was higher than that of both reading
difficulty groups.
Another finding was that
there were no differences in working memory between the "no
discrepancy" and the "discrepancy" groups. This finding is cited by Prof. Tannock as
supporting the argument that the IQ-achievement criterion is not suitable for
LD diagnosis.
A closer look at this study
shows that the children were assigned into the "no discrepancy" and
"discrepancy" groups not according to their IQ score but according
to…the simultaneous processing index of the KABC test. Almost all tests in this index (except matrix
analogies) measure visual processing in tems of CHC abilities (Alfonso et al, 2005). Thus, the children were assigned to "no
discrepancy" and "discrepancy" groups not by their IQ scores but
according to their visual processing scores.
Thus it can't be concluded
from this research that there's no difference in working memory between
children with and without IQ-achievement discrepancy.
What can be concluded is
that among children with poor reading there is no difference in working memory
between children with average visual processing and children with poor visual
processing. And why would there be? Children with good visual processing will
not always have good working memory as well.
Although both working memory and visual processing are parts of general
intelligence, and there should be a positive correlation between them, the size
of the correlation may be too low to cause a significant difference between the
groups in working memory.
Why is the working memory
of the reading difficulty groups poorer than that of the control group of
children without reading difficulties?
Since short term memory may be one of the causes of reading
difficulties.
Prof. Tannock further
writes:
".. our review of studies of
predictive validators also failed to find robust evidence of difference between
those with and without an IQ-achievement discrepancy. The groups do not differ
in long-term prognosis, nor do they differ in terms of response to intervention."
It would be interesting to
read the papers which looked at the long term prognosis. Is it long term prognosis of reading or of
the child's general wellbeing? It's
reasonable to assume that a child with reading difficulties and high IQ, say
120, will have a better educational and vocational outlook than a child with
reading difficulties and low IQ, say 75.
Our review of literature from the past
two decades concurred with several previous reviews: there is no robust
evidence to support the validity of this criterion. The criterion is
conceptually and statistically flawed.
The IQ-achievement discrepancy
criterion has its disadvantages (for example "wait to fail" in young
children; adolescents failing to achieve a discrepancy despite being leaning
disabled). Abandoning this criterion
because of the argument that there is no difference in cognitive processing
between children with and without a discrepancy or because of the argument that
children with and without a discrepancy have the same long term prognosis – is
not serious in my opinion.
The Work-Group reviewed the literature to
seek validation of three major approaches that have been proposed:
i)
inclusion of cognitive processing deficits (e.g., Hale, 2010; Kavale & Forness,
2000); Hale 2010 is an expert white paper consensus
written by 58 of the best researchers in the LD field. They agreed that "To meet SLD statutory
and regulatory requirements, a “third method” approach that identifies a
pattern of psychological processing strengths and weaknesses, and achievement
deficits consistent with this pattern of processing weaknesses, makes the most
empirical and clinical sense".
ii) ‘response-to-intervention’ criterion
(e/g. US Department of Education, 2004; Vaughn & Fuchs, 2003);
and iii) augmentation of the
low-achievement-for-age component of the DSM-IV criterion (e.g., Dombowski et
al., 2004; Tumney & Greaney, 2009).
I'll write here only about the
first suggested criterion (Prof. Tannock writes about all three).
Cognitive processing deficits as a possible
diagnostic criterion
The underlying premise is that various
cognitive (psychological) processing deficits play a causal role in SLD and
therefore would serve as valid indicators of SLD. The presumption of underlying
cognitive processing deficits is part of the IDEA (2004) definition of SLD, and
this approach is strongly supported in the field of neuropsychology as well as
by advocacy groups (e.g., Hale et al., 2010).
Prof. Tannock describes
critique of this approach and then presents the results of the literature
review of the work group concerning this issue.
One major critique of this approach is
that ‘processing deficits’ are rarely measured directly, but inferred from
scores on various neuropsychological tests, which in turn measure a complex web
of cognitive, behavioral, and motivational processes. This
is true. We can't get every child into a
brain scanner.
A second major critique is the limited
empirical support for the inclusion of cognitive processing deficits in the
diagnostic criteria for SLD. For instance although a meta-analysis found
moderate to large effect sizes for differences in cognitive processing between
children with SLD and typical development, there was no evidence that cognitive
deficits contributed to differential diagnosis of SLD).
It's
true that a person who has a cognitive disability does not necessarily have
LD. A person can have poor visual-spatial
processing, for example, and will find it difficult to navigate to new
places. But he may read, write and do
math well. A person who has good
reading/writing/math is not learning disabled.
Thus a specific cognitive disability is not a sufficient criterion for
LD. But many researchers think that a
specific cognitive disability is a necessary criterion for LD. IDEA 2004 is written according to this view.
Cognitive processing skills cannot be
used to rule in or rule out a diagnosis of RD, because the relationship between
the cognitive processing skills and reading skill is probabilistic and not
deterministic (i.e., not diagnostic).
By
contrast, the Work-Group’s review of the literature on predictive validators
revealed quite strong support for the inclusion of cognitive processing
deficits as a diagnostic criterion for SLD. For
example, several studies report that measures of cognitive processing skills
associated with reading (e.g., phonological awareness, naming speed) predicted
response to intervention in children with RD (Al Otaiba & Fuchs, 2002;
Frijters et al., 2011); Fuchs et al., 2012).
However, counter arguments include:
i)
cognitive deficits associated with RD are not unique to this disorder but are
shared with other neurodevelopmental disorders, such as ASD, ADHD, and
developmental coordination disorder (e.g., Willcutt et al., 2010);
ii) cognitive processing deficits that
underlie other manifestations of SLD
(mathematics, written expression) remain unclear (Geary, 2010; Ramus
& Hissar, 2012);
and iii) the required assessment of
cognitive processing skills may be prohibitively expensive and waiting lists
are often long (Compton et al., 2012).
Thus the Work-Group ruled out cognitive
processing deficits as a possible diagnostic criterion for SLD based on
consideration of its empirical evidence and clinical utility.
That's too bad! I find the last three counter arguments
un-persuasive. In doing so, the work
group has weakened LD diagnostic criteria in DSM5. The new criteria widen the boundaries of the
LD group to include people who are not LD.
Children who have average
achievement in reading/writing/math can be learning disabled according to DSM5
if their achievement "is sustainable only by extraordinary high levels of
effort or support". Thus LD is
diagnosed according to subjective judgement of the extent of effort that
the child invests to reach average achievement.
Admittedly, this has nothing to do with the cognitive ability criteria.
On the other end of the
scale, children with an IQ score of, say, 66, can also be diagnosed as LD
according to DSM5, although clearly their reading/writing/math difficulties are
caused by many cognitive disabilities that lower their general intelligence and
significantly interfere with everyday academic and other functioning. These are "SLOW LEARERS" in
Flanagan's conceptualization. These are
not children with a specific cognitive disability that interferes with learning
in a limited way, as LD children are meant to be.
The inclusion of these
groups causes improper spending of funds.
Slow learners need much more financial resources than LD children or
children who have "average achievement sustainable by high levels of
effort or support".

Considerations at the base of DSM5 learning disorder criteria - part A

Prof.
Rosemary Tannock was a
member of the DSM-5 Work-Group for ADHD and Disruptive Behavior Disorders and a
Liaison member of the Neurodevelopmental Disorders Work Group to advise on
Learning Disabilities.
In this
paper she describes the considerations that lead to the decisions that were made regarding LD diagnostic criteria. I'll
present her views in black or orange, and thoughts of mine in green.
This post
does not "cover" all the aspects the paper discusses, but rather
focuses on the things that were interesting to me. Because of the post's length, it's divided
into two parts. This is part A.
"DSM-5, the fifth edition of the
American Psychiatric Association’s Diagnostic
and Statistical Manual of Mental Disorders, was published in May 2013, amidst
a storm of controversy. As with the
previous versions, this new volume has received strong criticism from the
public, individuals with LD and their families, policy makers and well as from
clinicians and researchers in the field of mental health.
One of the ongoing concerns was that this
version of the DSM continues to reify the concept of ‘discrete disorders’ of
the mind, based on various sets of observable signs and symptoms, rather than
on specific biological tests or atypical patterns of brain-based states. However, since scientific knowledge had not
yet advanced enough to use neuroscience and genetics to shape the
conceptualization of mental disorders in DSM-5, the diagnostic criteria were to
remain as behavioral descriptors.
The DSM has a broad influence not only on
the diagnosis of mental disorders, but also on how they are perceived by the
public, healthcare personnel, employers, and those in the school and judicial
systems; how they are funded by medical insurance agencies; how the public
policy for mental health is maintained or changed; and how research agendas are
set.
Significant
changes in diagnostic criteria undermine our ability to rely on research done thus
far. All previous learning disabilities
(LD) research was done in light of the old criteria. Research subjects were divided into LD groups
and control groups according to the old criteria, thus conclusions about
differences between LD and non – LD persons are valid for people diagnosed in
light of the old criteria.
LD is a "disorder" in DSM5,
since medical conditions are called disorders rather than disabilities:
‘Learning Disabilities’ is an educational term.
I still wonder why LD
should appear in the DSM and why it is considered to be a "medical
condition".
The DSM-5 Manual advises that the
diagnostic criteria “are offered as guidelines for making diagnoses, and their
use should be informed by clinical judgment.”
The
field lacks a complete understanding of LD: instead there are burgeoning
descriptions, guises, guesses, hypotheses, and controversies. To date, there is
no international consensus as to what constitutes LD, its operational
definition (diagnostic criteria), or who can or cannot conduct the required
assessment or make the diagnosis.
Early perspectives of LD, in the 1960's,
recognized that impaired academic skills (reading, arithmetic) occur in the
context of average or even superior intellectual abilities. Other important
insights from these early perspectives include the notion that LD (dyslexia)
was congenital, heritable, and manifested primarily by males. Underlying cognitive deficits
(possible causal factors) focused initially on visual problems. For
example, Hinshelwood (1917) postulated that the primary disability of children
with word blindness was in visual memory for letters and words and that it was
an inherited condition.
According to the U.S. Federal Law (IDEA,
2004, 20 USC Sec. 1401 (30, SLD), LD is defined as “a disorder in one or more basic psychological
processes involved in understanding or using language, spoken or written, that may manifest
itself in the imperfect ability to listen, think, speak, read, write, spell, or
d arithmetical calculations.” Thus the notion of underlying
deficits in cognitive processing as causal factors was instantiated in the conceptualization
of LD. CHC
abilities are of course cognitive abilities and thus a disability in one or
more CHC abilities is one of the criteria for LD diagnosis not only according
to Flanagan but according to the US federal law as well.
This prevailing legal definition
restricts the concept of LD to a language-based disorder, but does not include
non-language-based learning difficulties, such as dyscalculia, as defined by
some researchers. Motor skills are also not included. It does seem like
language gets in this definition a role more central than it should take. Difficulties in
thinking/reading/ writing/ arithmetic etc. can have many other causes beyond
language.
According to DSM-5, SLD (specific
learning disorder) is a type of Neurodevelopmental Disorder that impedes the
ability to learn or use specific academic skills, such as reading, writing, or
arithmetic, which serve as the foundation for other academic learning. The
learning difficulties are ‘unexpected’ in that other aspects of development
seem to follow a typical trajectory, or are only minimally delayed. Early signs
of learning difficulties may be discernible in the preschool years (e.g.,
difficulty learning names of letters or to count objects), but they can only be
diagnosed reliably after starting formal education.
In DSM-5, SLD is understood to be a
chronic condition that typically persists into adulthood, albeit with changes
in the way the learning difficulties manifest. In part, this is because adults
learn compensatory strategies (e.g., avoid reading by using other media to
obtain information or for calculation; use specialized software to assist with
reading/writing activities or calculators to assist with numerical
activities). Adults
can also use non-technical compensatory strategies like the ability to
comprehend what they read despite inaccurate decoding due to better use of
general knowledge and deduction skills.
The causes of SLD are unknown, but
research suggests the learning difficulties run in families, are heritable, and
involve interplay of both genetic anomalies and environmental factors (e.g.,
prematurity, prenatal exposure to neurotoxins from tobacco, alcohol, street
drugs, or other environmental toxins). Neither are the underlying mechanisms of
SLD known, although both neural and psychological accounts exist, particularly
for difficulties learning to decode words.
Neuroimaging studies have revealed alterations in both structure and
function, but it is unclear whether these brain differences are a cause,
consequence or correlate of SLD.
A
developmental history of Speech Sound Disorder and/or Specific Language
Impairment in preschool years is a common precursor of all three LD categories
listed in DSM-IV-TR (reading, writing, mathematics), but particularly for poor
skills in reading comprehension, spelling, arithmetic fact retrieval, and
calculation. Moreover, one longitudinal
study of a community-based sample of children identified with pervasive
speech/language disorders in kindergarten were found to have an estimated 3- to
6-fold greater risk for LD (all categories alone or in combination) in young
adulthood compared to typically-developing youngsters.
Longitudinal studies provide strong evidence
of a developmental accumulation of learning difficulties with increasing
cognitive demands of the curriculum. For instance, children with speech sound
disorders in early childhood, later manifest difficulties learning to read,
spell, and write in the school years.
Also, over 50% of children with phonologically-based reading
difficulties but no apparent difficulties in learning basic arithmetic at age 5
years, manifest learning difficulties in mathematics as well as continued
problems in reading at age 7. However,
intervention outcome studies provide no evidence that intervention for one
academic domain or its subskills transfer to other academic domains.

שעמדו בבסיס הגדרת לקות למידה

זהו חלק ב' של פוסט שדן במסמך שחיברה
פרופ' רוזמרי טאנוק, שהיתה חברה בקבוצת העבודה שחיברה את הקריטריונים לאבחון לקות
למידה ב – DSM5. אמשיך להציג את
עמדותיה בצבע שחור או כתום ותוספות/הרהורים
שלי עליהן בצבע ירוק.
בחלק זה פרופ' טאנוק מנסה להסביר מדוע קבוצת
העבודה ויתרה על הגדרת לקות למידה לפי "נוסחת הפערים" ב –5 DSM. לאחר מכן היא מנסה להסביר מדוע קבוצת העבודה
ויתרה גם על האפשרות להגדיר לקות למידה לפי ליקויים בעיבוד הקוגניטיבי/ביכולות
הקוגניטיביות.
על פי נוסחת הפערים, ילד לקוי למידה הוא ילד בעל הישגים
נמוכים בקריאה/כתיבה/חשבון ורמת משכל כללית תקינה ומעלה. כלומר קיים אצלו פער בין רמת ההישגים
בקריאה/כתיבה/חשבון לבין רמת המשכל הכללית.
כל זאת בתנאי שלא ניתן להסביר את ההישגים הנמוכים בקריאה/כתיבה/חשבון באופן
טוב יותר באמצעות אחד מגורמי ההדרה (בעיה חושית כמו כבדות ראיה, פיגור שכלי, מצב
רגשי, חוסר תפקוד לימודי, הוראה לא מתאימה, היות הילד עולה חדש ועוד). אם אחד מגורמי ההדרה הללו מסביר טוב יותר את
ההישגים הנמוכים של הילד בקריאה/כתיבה/חשבון, הסיבה לקשייו אינה לקות למידה.
על פי נוסחת הפערים, לקות למידה היא מצב בו יש לילד
קשיים קוגניטיבים ספציפים הגורמים להנמכה בקריאה/כתיבה/חשבון אצל
ילד שהוא בסך הכל נבון ויכול לתפקד בלימודים לא רע אם ילמד להתמודד עם הלקות או "לעקוף"
אותה. ילד כזה שקורא באופן לא מדויק ובקצב
איטי, יתקשה להבין את הנקרא, אבל יהיה מסוגל להבין היטב את אותו הטקסט אם הוא
יוקרא בפניו.
על פי נוסחת הפערים לקות למידה אינה מצב
בו יש לילד קשיים בתפקודים קוגניטיבים רבים. קשיים רחבים כאלה מורידים בדרך כלל את רמת
המשכל הכללית. ילד עם קשיים בתפקודים
קוגניטיבים רבים יתקשה "לעקוף את הלקות". ילד כזה שקורא באופן לא מדויק ובקצב איטי,
יתקשה להבין את הנקרא וגם יתקשה להבין את אותו הטקסט אם הוא יוקרא
בפניו. זאת מכיוון שהבנת הנשמע, הנשענת בין השאר על יכולת ההסקה (היכולת
הפלואידית) ועל הידע המגובש – עלולה להיות נמוכה אצלו.
פרופ' טאנוק כותבת כך:
הלוגיקה מאחורי הגדרת לקות למידה לפי פער בין רמת המשכל הכללית להישגים
בקריאה/כתיבה/חשבון היא שהגורמים לקשיים בלמידת קריאה/כתיבה/חשבון יהיו שונים בין
אנשים שיש להם פער כזה ובין אנשים שאין להם פער כזה.

IQ-discrepancy definition is that the cause of the learning difficulties would
differ between those with and without IQ-Achievement discrepancy.
אני לא מבינה זאת כך.
כפי שאני מבינה, אותן לקויות קוגניטיביות יכולות לגרום לקשיים
בלמידת קריאה/כתיבה/חשבון הן אצל אנשים בעלי רמת משכל ממוצעת (כלומר אנשים שיש להם
פער) והן אצל אנשים בעלי רמת משכל נמוכה מהממוצע (כלומר אנשים שאין להם פער). ההבדל בין שתי הקבוצות הוא שלאנשים שאין להם

אנליזות רבים נערכו כדי לבחון אם יש הבדלים בתהליכים הקוגניטיבים בין ילדים שיש
להם פער לבין ילדים שאין להם פער. נמצא באופן
עקבי ששתי הקבוצות אינן נבדלות בתהליכים הקוגניטיבים שלהן.

al., 2005; Francis et al., 2005; Siegel, 1992) and meta-analyses (Hoskyn, 2000; Maehler & Schuchardt, 2009;
Stuebing et al., 2002) have been conducted to test for differences between
discrepant and non-discrepant groups of children with LD in terms of cognitive
processes that contribute to learning. Findings are consistent: the two groups do not differ in their
cognitive processing skills".
הסתכלתי בחלק מהמחקרים המצוטטים כאן. המחקר של , Maehler &
Schuchardt, 2009 לדוגמה, מכונה על ידי פרופ'

בקבוצת הביקורת הילדים לא התקשו בקריאה. בשתי הקבוצות האחרות הילדים כן התקשו בקריאה.
באחת משתי קבוצות המתקשים בקריאה הילדים היו "עם
פער" (כלומר עם "רמת משכל ממוצעת") ובשניה הילדים היו "בלי
פער" (כלומר עם "רמת משכל נמוכה מהממוצע").
החוקרות מצאו שהזיכרון לטווח קצר של ילדי קבוצת הביקורת
היה גבוה משל הילדים שהתקשו בקריאה בשתי הקבוצות גם יחד ("עם פער"
ו"בלי פער").
ממצא נוסף היה, שלא היו הבדלים בזיכרון לטווח קצר בין
קבוצת הילדים "בלי פער" לבין קבוצת הילדים "עם פער". וממצא זה
שימש עבור פרופ' טאנוק לביסוס הטענה שנוסחת הפערים אינה מתאימה להגדרת לקות למידה.
אבל אם נסתכל בתשומת לב במחקר זה, נראה שהילדים חולקו
לקבוצות "עם פער" ו"בלי פער" לא לפי רמת המשכל הכללית אלא
לפי...אינדקס העיבוד הסימולטני במבחן קאופמן KABC. כל מבחני העיבוד הסימולטני (למעט אנלוגיות
צורניות) במבחן הקאופמן בודקים עיבוד חזותי במונחים של CHC. כלומר הילדים חולקו לקבוצות "עם פער"
בין רמת המשכל הכללית שלהם לבין הישגיהם בקריאה/כתיבה/חשבון ו"בלי פער"
כזה לא לפי רמת המשכל הכללית אלא לפי העיבוד החזותי.
לכן לא ניתן להסיק מהמחקר הזה שאין הבדל בזיכרון לטווח
קצר בין ילדים מתקשים בקריאה שיש להם פער ושאין להם פער.
מה שניתן להסיק הוא שאין הבדל בזיכרון לטווח קצר בין
ילדים מתקשים בקריאה שהם גבוהים בעיבוד חזותי ולבין כאלה שהם נמוכים בעיבוד חזותי. ולמה שיהיה?
ילדים גבוהים בעיבוד חזותי לא תמיד יהיו גבוהים גם בזיכרון לטווח
קצר. אמנם הן הזיכרון לטווח קצר והן
העיבוד החזותי בודקים את המשכל, ולכן צריך להיות ביניהם מתאם חיובי כלשהו, אבל הוא
עשוי להיות נמוך מכדי לגרום להבדל משמעותי בין שתי הקבוצות בזיכרון לטווח קצר.
למה הזיכרון לטווח קצר של שתי קבוצות הילדים המתקשים
בקריאה ביחד היה נמוך מזה של ילדי קבוצת הביקורת שלא התקשו בקריאה? מכיוון שזיכרון לטווח קצר עשוי להיות אחד
הגורמים לקשיים בקריאה.
פרופ' טאנוק ממשיכה וכותבת:
קבוצות של ילדים עם פערים וללא פערים לא נבדלות בתגובה להתערבות, וגם
לא בפרוגנוזה לטווח ארוך.
מעניין לקרוא את המאמרים שבדקו את הפרוגנוזה לטווח
ארוך. האם מדובר בפרוגנוזה של הקריאה בלבד
או בפרוגנוזה של מצבו הכללי של הילד לטווח ארוך?
סביר להניח שילד עם קשיים בקריאה ומשכל מעל לממוצע, למשל 120, יהיה בכל זאת
בעל פרוגנוזה להצלחה טובה יותר בלימודים ובתעסוקה מאשר ילד עם קשיים בקריאה ומשכל
שמתחת לממוצע, למשל 75.
כך, סקירת
הספרות מראה שאין עדות חזקה ומוצקה התומכת בתוקף של פער בין רמת המשכל הכללית
להישגים בקריאה/כתיבה/חשבון כקריטריון ללקות למידה. הקריטריון הזה הוא שגוי באופן המשגתי וסטטיסטי.
לנוסחת הפערים יש אמנם חסרונות (שדווקא אותם פרופ' טאנוק לא מפרטת),
אך לפסול אותה בשל הטיעון שאין הבדל בתהליכים הקוגניטיבים בין ילדים עם פערים לבין
ילדים ללא פערים, או בשל הטיעון שלילדים עם פערים ובלי פערים יש אותה פרוגנוזה
לטווח ארוך – זה לא רציני לדעתי.
באיזה
קריטריון להשתמש במקום הקריטריון הזה? עלו
בפני קבוצת העבודה שלוש הצעות:
א.
קריטריון של ליקויים בעיבוד קוגניטיבי (הנמכות
ביכולות הקוגניטיביות, כפי שנהוג בהגדרה של פלאנגן ללקות למידה, על פי CHC). קריטריון זה הוצע על ידי Hale, 2010; Kavale & Forness, 2000. Hale 2010 הוא נייר
עמדה של 58 מטובי המומחים בתחום לקויות הלמידה.
מומחים אלה הסכימו ביניהם ש"הגישה ההגיונית ביותר מבחינה אמפירית וקלינית לזיהוי לקות
למידה ספציפית היא גישה שמזהה חוזקות וחולשות בתהליכי עיבוד פסיכולוגים, וחסכים
בהישג שעולים בקנה אחד עם חולשות אלה". פוסט שכתבתי על נייר עמדה זה מופיע כאן.
ב. קריטריון של תגובה להתערבות.
ג. קריטריון של הישג נמוך בקריאה/כתיבה/חשבון ביחס
למצופה בגיל הילד.
בפוסט זה אתייחס רק לקריטריון הראשון (פרופ' טאנוק מפרטת גם על שני
הקריטריונים האחרים).
חסכים בעיבוד הקוגניטיבי כקריטריון
דיאגנוסטי אפשרי
ההנחה
העומדת בבסיס הרעיון הזה היא שלקויות שונות בעיבוד הקוגניטיבי גורמות לקשיים
בקריאה/כתיבה/חשבון ולכן יכולות לשמש כסמנים של לקות למידה. הנחה זו היא חלק מהגדרת IDEA 2004
ללקות למידה, והיא מקובלת מאד בשדה הנוירופסיכולוגיה.
פרופ'
טאנוק פותחת בתיאור ביקורות על הנחה זו, ולאחר מכן מציגה את תוצאות סקירת הספרות
שערכה הועדה בנושא זה.
ביקורות
על הנחה זו:
ביקורת עיקרית אחת על הנחה זו היא ש"לקויות
העיבוד" רק לעתים רחוקות נמדדות באופן ישיר, אלא בדרך כלל מוסקות מציונים
במבחנים נוירופסיכולוגים שונים. אכן אנחנו לא עושים
לכל ילד הדמיה מוחית כדי לבדוק לקויות בעיבוד קוגניטיבי.
ביקורת עיקרית שניה היא תמיכה מחקרית מוגבלת לשימוש
בקריטריון זה לאבחנה של לקות למידה. למשל,
למרות שמטא – אנליזה מסויימת מצאה שיש הבדלים משמעותיים בעיבוד הקוגניטיבי בין
ילדים עם לקויות למידה לבין ילדים עם התפתחות תקינה, לא נמצאה עדות לכך שהלקויות הקוגניטיביות
תרמו לאבחנה מבדלת של לקות למידה. בנוסף, מחקר
רחב היקף בארה"ב ומחקר אורך בינלאומי מצאו, שמנבאים קוגניטיבים שחוקרים
מאמינים שיש להם תפקיד סיבתי בדיסלקסיה הם לא הכרחיים ולא מספיקים לאבחנה של לקות
למידה בקריאה.
אכן, עצם העובדה שיש לאדם לקות ביכולת
קוגניטיבית מסויימת, אין פירושה שאותו אדם הוא לקוי למידה. אדם יכול להיות לקוי בעיבוד חזותי – מרחבי,
למשל, ויתקשה מאד לנווט למקומות חדשים.
אבל הוא עשוי לתפקד באופן תקין בקריאה, בכתיבה ובחשבון. אדם שמתפקד באופן תקין בקריאה, כתיבה וחשבון
אינו לקוי למידה. כך, לקות ביכולת קוגניטיבית
אינה קריטריון מספיק לקביעת לקות למידה.
אולם חוקרים רבים סבורים שלקות ביכולת קוגניטיבית היא כן קריטריון הכרחי
לקביעת לקות למידה. לכן קריטריון זה נכלל בהגדרת לקות למידה בחוק האמריקני.
כישורי
עיבוד קוגניטיבי אינם יכולים לשמש כדי לקבוע או לשלול אבחנה של לקות
בקריאה, מכיוון שהקשר בין כישורי עיבוד קוגניטיבי וכישורי קריאה הוא הסתברותי ולא
דטרמיניסטי (כלומר, לא דיאגנוסטי).
בניגוד לעמדה
שהוצגה בספרות שהוצגה עד כאן, סקירת הספרות של קבוצת העבודה של ה – 5DSM תומכת בהצעה לכלול לקויות בעיבוד
הקוגניטיבי כקריטריון דיאגנוסטי ללקות למידה.
מספר מחקרים דיווחו שמדדים של כישורי עיבוד
קוגניטיבי הקשורים לקריאה (למשל, מודעות פונולוגית, מהירות שיום) מנבאים תגובה
להתערבות אצל ילדים עם לקות בקריאה.
אולם הועלו טיעונים כנגד עמדה זו:
א.
לקויות קוגניטיביות נמצאות גם בהפרעות נוירו-התפתחותיות אחרות, כמו ASD, adhd והפרעת קואורדינציה
התפתחותית.
ב. לא
ברור אילו לקויות בעיבוד קוגניטיבי עומדות בבסיס קשיים במתמטיקה ובהבעה בכתב.
ג.
ההערכה הנדרשת של כישורי עיבוד קוגניטיבים עשויה להיות יקרה והתורים לאבחון
יהיו ארוכים.
מסיבות אלה (שאף אחת
מהן אינה משכנעת לדעתי) קבוצת העבודה פסלה את הלקויות בעיבוד הקוגניטיבי
כקריטריון דיאגנוסטי ללקות למידה.
חבל מאד! בעשותה זאת החלישה הועדה מאד את הגדרת לקות
למידה על פי 5DSM. הגדרה חדשה זו מרחיבה
מאד את קבוצת הילדים לקויי הלמידה וכוללת בתוכה אוכלוסיות שאינן לקויות.
ילדים שמגיעים להישגים תקינים
בקריאה/כתיבה/חשבון יכולים להיות מוגדרים כלקויי למידה על פי DSM5 אם "ניתן לשמר הישגים
אלה רק באמצעות רמות גבוהות של מאמץ או תמיכה". כך לקות למידה נקבעת על פי שיפוט סובייקטיבי
של מידת המאמץ שהילד משקיע כדי להגיע להישגים התקינים.
בקצה השני של הסקאלה, ילדים עם רמת
משכל 66, למשל, יכולים אף הם להיות מוגדרים כלקויי למידה על פי DSM5, למרות שברור שקשייהם בקריאה/כתיבה/חשבון נגרמים בשל לקויות
קוגניטיביות נרחבות, ביכולות רבות, שגורמות לרמת המשכל הכללית הנמוכה, ופוגעות
באופן רחב במגוון של תפקודים לימודיים ואחרים. אלה ילדים שפלאנגן מכנה SLOW LEARNERS. אלה לא ילדים שיש להם לקות קוגניטיבית
ספציפית שפגיעתה בתפקודים הלימודיים והאחרים היא מוגבלת, כפי שאמורים להיות ילדים
לקויי למידה.
הכללתן של שתי קבוצות אלה גורמת
להקצאה לא נכונה של תקציבים. ילדים שהם SLOW LEARNERS זקוקים לעזרה אינטנסיבית הרבה
